I tried searching, but it wouldn't give me any results because it's c is a common letter. I asked the Dr's office if there was any paperwork I need to bring any paper work, they said they had it all there. This is a US Dr. From what I understand it only affects where the exam goes?

Sorry I wasn't more clear, I had the exam but don't remember if appendix c was in all the paper work. The Dr mails that in with the exam, correct?
Not sure exactly what it is the doctor sends in - but YOU need to send in the receipt from the medical - there's a number on there they use to match up the results.

I brought that form when we went to the Dr and they said they didn't need it so all I sent was the reciept that she game me. When I reached the call center they said that's fine.
As long as the DMP knew which visa office would be processing your file, they should know which Regional Medical Centre to send the results to. The visa office will request the results from the RMC when they're ready to review them. And don't worry, if they can't find them they'll let you know and then you can track them down yourself - either through the DMP or through the RMC, and get them sent along.
Thank you. I just wanted to make sure it's possible to correct it. There was so many forms, but I'm pretty sure I put in Buffalo in one of them. Theres so much paperwork! LOL
