I know this is probably yet another dumb question from me, but I was adopted by my biological mother's parents soon after I was born. I do know why she gave me up and who the father is and all that, so do I need to include this info via supplemental sheets on my app? :) If I do, I assume that I need to include my adoption papers as well?

Thanks for your time!
I'm assuming you're talking about family members to include in Form 5406. I haven't ever seen this come up before, but I don't think there's any requirement that you disclose information about your biological parents. Your adoptive parents are your legal parents, so their names would be included in the space for mother and father on Form 5406.
Robsluv is probably correct.

If, however, your birth certificate bears the name of your biological parents (that is, if there is any *apparent* discrepancy, for any reason, between who you list as parents and any documentation you will be submitting), I'd lean toward explaining that, as you say, on a supplemental page. I don't think I would include copies of documentation to show that, as just the explanation should be sufficient. Remember, their first priority in this particular information is to positively identify the applicant, and then to have some information with which they can assess the applicant's background and to identify familial relations that might indicate ineligibility or reasons for fraud. While adopted applicants themselves (as opposed to inclusion or sponsoring of adopted children) is not an issue one sees much on the boards about immigrating to Canada, I am confident it is very common, something they are accustomed to seeing, not at all problematic -- probably common enough that they would say up front if any additional documentation was required.
*Thumbs up* Thumbsup Thank you both! I wasn't sure if that would be a problem or not. My adoptive parents are on my birth certificate, so there we go! Bliss
