Just a head's up that the New York office responsible for police clearances are NOT answering the email address listed on their website. Unfortunately, I learned this the hard way.
To request the application package, be sure to phone them and speak with them in person.
This must be a very recent development. I requested mine by email about a month ago and got it

Wait.. I'm confused. Is this for NY city police clearance or the NY state police clearance??
I thought for NY state police clearance, you only need to go to L-1 livescan fingerprinting (make appt online) and then they'll process it??? (here's the NYS info:

Should I be getting the certificate from NYPD or from the NY state?.. So confused..

As for the clearances, they're supposed to be state clearances (if you even need them, normally applicants don't ever get asked for these). I didn't have to go through this myself, but the link doghouse posted above appears to be the correct one for NY state (minus the end bracket that breaks the link :P).

All the instructions are on that site. But, again, US state clearances are NOT required and do not have to be sent in with your application. It is very rare that Buffalo decides they need them, and even then they will specifically ask you for them if it turns out they do. Most applicants who lived in the States long enough to need police clearances only ever have to submit the FBI clearance.

I just called the L-1 fingerprinting services and they said that whatever the reason code was that they will submit it to the appropriate location. Since my reason code was immigration they said it would be sent there. Don't I need the results so I can submit it with my application??

Yes, you need to receive the results.
My husband had his fingerprints for the NY state clearance done by L1 last November. We had the results sent directly to us and received them within a week.
There are cases that the results would be sent directly to employers, etc, but in this case you need to stress you want them sent to you. NY state just started using the L1 service in the last year. Even when my husband went last November he had to explain that the results needed to come to him.
