We have to fill in the form to exclude my husband's son as we can't easily contact his son's mother who has full custody, so unfortunately we're choosing to exclude him. The form has a line on it for a witness though and we don't know if we have to have a notary witness it or if anyone can witness it. Thanks for your help in advance.
