Hi Everyone,
I just received my CAIPS notes today but something doesn't seem right.
I sent the request for CAIPS notes on Feb 6,2008 to CIC Ontario Region at 25 St.Clair East. What I received seems to be a faxed copy that the Canadian Consulate at Buffalo seemed to have sent to this office. The fax information on the top shows 10 pgs in total, but I only received pages 4-10 in my envelope. Is this normal?
I dont have pages that show information about my "Case Status", "Documents", "Medical Case Directory" or "Immigration Assessment Record Summary". Starting from page 4 all the way to page 10, I seem to have got all the Visa Officer's notes and the progress of my file.
Just to give you'll the last few progresses of my file so far has been:
Completed Medicals for myself and my wife, Provided Police Clearance, Paid Right of Landing fees.
Could you please help me decipher these last few comments?

21-NOV-2007: FILE W/ SM FOR REVIEW

They received the police clearance certificate for Russia - no criminal record.
21-NOV-2007: FILE W/ SM FOR REVIEW
File with "SM" (presumably initials of the officer) for review.

They assigned a
Brought Forward date to the file - 21 March 2008 - this is when the file will next be looked at. Presumably they are still waiting for something, possibly background / security checks.

Thanks a bunch Matthew for the explanation. I was guessing as much that those would be the interpertation of the notes.
What about the missing pages? Do you think I should write back and request them?
Along with the CAIPS notes I got a letter from the Access and Privacy Officer. Here's an extract from it.
This is in response to your request for personal infrmation made under the Privacy Act, which was received by CIC, Ontario Region.
Enclosed, please find a copy of most of the information to which you are entitled to pursuant to Section 12(1) of the Privacy Act.
The remaining documents have been forwarded to the Manager, Access to Information and Privacy, Ontario Region for further consideration.
Could this mean, that further documentation will be sent to me at a later date?

Exactly. They sent the remaining pages to be reviewed by someone higher up, because there's information on there they aren't sure if you are allowed to have.
Most likely, you'll get the other pages back in a few weeks, with every instance of the word "CSIS" carefully blanked out, yet still completely obvious from the context that that's what it said.

I received a follow-up correspondance from CIC, Ontario Region. The only update they provided me is my Medical Case Directory. Here are the details.
Myself MED REQ'D: Y, MED STAGE: M01, VALIDITY: 21-06-2008
Spouse MED REQ'D: Y, MED STAGE: <BLANK>, VALIDITY: <BLACK>
I was confused why my wife's medical information was blank, as she completed her Medicals in August 2007 in Russia, I had completed my Medicals at the same Validity date in 2007 in Toronto.
Then I read the letter that came along with the correspondence and things still SEEMED unclear. Here are the important parts of it...
I have had an oppotunity to review the information withheld by Ms.XXXX. As a result, I am pleased at this time to be able to release one additional document (it was the Medical Case Directory) to which you are entitled under Section 12(1) of the Privacy Act. Certain information contained therein qualified for exemption pursuant to Section 26 of the Privacy Act. A copy of the relevant section of the Act is enclosed for ease of reference. The only remaining document will be forwarded to the Access of Information and Privacy Division, National Headquaters in Ottawa, for their consideration.
You may send a request concerning the outstanding document to the following address:

Here's Section 26 of the Privacy act that was attached...
26. The head of a government institution may refuse to disclose any personal information requested under subsection 12(1) about an individual other than the individual who made the request, and shall refuse to disclose such information where the disclosure is prohibited under section 8.
When I sent the request for the CAIPS notes, I mentioned my name in the request. Is this the reason they're with holding the remaining information (i.e. "Case Status", "Documents", "Immigration Assessment Record Summary") because my wife is currently residing in Russia and I need to send a seperate CAIPS request for my wife along with the $5? I thought I didnt need to as we were under one application.
Or could it mean that they didnt receive my wife's medicals yet and that is delaying our application. How do I find out if they received my wife's medicals, as my bring forward date is March 21,2008 and I want to avoid waiting for a another date in the future.

The reason is that you can only get information about yourself, unless you send in this form signed by your wife, giving permission for the government to give you information about her:

Even though you are her husband, CIC cannot legally release her personal information to you without her permission. So, if you want info on your wife's medicals, you'll have to re-request CAIPS and include that form.
The other info that was sent to Ottawa is most likely pages that contain info about background checks. You'll probably get those back in another few weeks, with every reference to "CSIS" blanked out.

I will go ahead and send the form you linked for my wife as well.
I'm still confused though, if they didn't want to provide information on my wife, why did they provide me with the CAIPS notes that contained the Immigration offer's notes of the progress of my application; including information that they received the Police Clearance of my wife.
Also, did they intentionally blank out the Medical information for my wife? or did they really not receive my wife's medical?
I guess the only way to find out is to request the CAIPS notes again for her.

The DMPs should send medl results to RMO (Regional medl Office) in-charge of that location & NOT to cic (

Thou you both have taken medls at diff countries, ur medls should be sent to RMO, Ottawa b'se thats where YOU, the Principal applicant had taken medls.. But the DMP at Russia might have sent it to RMO in-charge of Russia instead of Ottawa & hence it might NOT have still reached/uploaded to CIC..You NEED to follow-up for sure
1. Contact the DMP in Russia; find out when & where the medl results of ur wife had been sent
2. You can also contact Ottawa at
613-941-6641, 613-941-2170 & politely request if they have medl results of ur entire family.. Remember, its NOT a public number & ottawa ppl r NOT obliged to answer.. But when u explain ur concerns very politely, they might answer whether or NOT they have all medl results.. (Nothing more than that)
3. As a last resort, u can email CIC & ask them if they have medls of the entire family..

Thanks for your reply.
I spoke to my wife who spoke to the DMP in Russia where she undertook her medicals. The DMP confirmed that they sent the medical results few days after she completed her medicals to London, which I presume is the RMO for that location as also seen in the link you posted.
The DMP also added that if CIC didn't receive the medical results from the RMO, they would need an official letter from CIC Buffalo stating that the medicals were not received and requesting the DMP to send them again.
Now I have the two other options to find out if they've received her medicals, one is to contact Ottawa by phone and secondly emailing CIC.
I guess a third option could be that I could request the CAIPS notes on behalf of my wife and myself together and hopefully they contain the updated medical information.
