When my husband goes for hios medical does he have to bring any forms with him? We've only sent for the FBI and State checks. I'm waiting for the Sponsorship application to be mailed out to me.

Why don't you just get the application forms off the CIC website? Yes, there is a form he needs to take with him to the medical, but which one depends on whether he's applying inland or outland. Because you mention FBI and State criminal checks, I'm assuming he's from the States. In 99.9% of cases US citizens (even ones staying with their spouses in Canada) should be applying outland. The form you need for an outland medical is in Appendix C of the US Region Specific guides.

The only form I took with me to my medical was Appendix C (fyi I am applying outside, but medicals done inside). The doctor provided the remaining paperwork necessary to complete all of my medical tests.
